예: OO 웰니스 스튜디오 회원 가입 신청서
예: OO 웰니스 스튜디오(이하 "센터")과 홍길동(이하 "회원")은 아래 규정에 동의하고 회원 등록을 신청합니다. 예: OO 웰니스 스튜디오 (hereinafter "Center") and Member 홍길동 agree to the following terms and apply for membership registration.

제1조 (레슨 정책)

  1. 약속된 레슨 시간에 연락 없이 불참 시(당일 취소 포함) 해당 횟수가 차감됩니다. Failure to attend or same-day cancellation without prior notice will result in session deduction.
  2. 취소·변경은 전일 21시 이전까지 강사에게 전달해야 횟수가 차감되지 않습니다. Cancellations or changes must be communicated by 9:00 PM the day before to avoid deduction.
  3. 회원권 양도는 센터의 사전 동의가 필요합니다. Membership transfer requires prior consent from the center.

제2조 (회원권 사용 기간)

  1. 등록 횟수별 기본 유효 기간: Default validity period by number of sessions registered:
    • 10회: 등록일로부터 2개월 — 10 sessions: 2 months
    • 20회: 등록일로부터 4개월 — 20 sessions: 4 months
    • 30회: 등록일로부터 6개월 — 30 sessions: 6 months
    • 50회: 등록일로부터 10개월 — 50 sessions: 10 months
  2. 유효 기간 내 미소진 횟수는 자동 소멸됩니다. Unused sessions expire automatically at the end of the validity period.
  3. 질병·부상 등 불가피한 사유로 장기 미수강 시, 사전 요청에 한해 기간 연장이 가능합니다. Extensions may be granted for unavoidable reasons such as illness or injury, provided prior notice is given.
  4. 이번 등록 기간: 시작일 ~ 만료일

제3조 (환불 규정)

소비자분쟁해결기준(공정거래위원회 고시 제2023-2호) — 체육시설업 기준 준용

  1. 환불은 공정거래위원회 고시(체육시설업 소비자분쟁해결기준)에 따라 처리됩니다. Refunds are processed per the Korea Fair Trade Commission guidelines for fitness businesses.
  2. 계약 체결 후 이용 전(계약일로부터 7일 이내) 전액 환불 가능합니다. Full refund available within 7 days of signing if no sessions have been used.
  3. 이용 개시 후 환불 기준: Refund policy after sessions begin:
    • 등록 횟수의 1/3 미소진: 소진 횟수 × 단가 110% 공제 후 잔액 환불 Less than 1/3 used: deduct used × unit price × 110%, refund remainder
    • 등록 횟수의 1/2 미소진: 소진 횟수 × 단가 120% 공제 후 잔액 환불 Less than 1/2 used: deduct used × unit price × 120%, refund remainder
    • 등록 횟수의 1/2 초과 소진: 환불 불가 More than 1/2 used: no refund
  4. 당일 취소·무단 결석은 소진된 횟수로 간주합니다. Same-day cancellations or no-shows count as used sessions.

제4조 (기타 규정)

  1. 귀중품 보관 / Valuables — 센터 내 분실·도난에 대해 센터는 책임지지 않습니다. The center is not responsible for loss or theft of personal belongings.
  2. 강사 변경 / Instructor Change — 담당 강사의 부재·퇴직 시 동급 이상의 강사로 교체될 수 있습니다. A substitute of equal or higher level may be assigned.
  3. 안전사고 / Safety — 수업 시간 외 본인 부주의로 발생한 부상에 대해 센터는 책임지지 않습니다. The center is not liable for injuries caused by member negligence outside lesson hours.
  4. 시설 이용 / Facility Use — 기기·시설 사용 시 강사의 안내에 따르며, 파손 시 수리비를 부담합니다. Follow instructor guidance when using equipment; members are liable for any damage caused.
